Love the bit about taking cords etc off the prisoner without adequate rationale. It might be worth poking the idiots who wrote this report with a sharp stick to remind them that the reason is to prevent said prisoner killing themselves and thereby creating a shit storm of finger pointing and blaming the custody staff for allowing the prisoner to be able to kill themselves. Really where do they get these people from?
